障害者に対する歯科診療が自律神経機能に及ぼす影響 : 心拍変動解析を用いた評価

Abstract

 障害者に対して歯科診療を行う場合,精神的,身体的侵襲を評価することが困難なことがある.近年,心拍変動解析を用いて自律神経機能をリアルタイムに評価できるモニターシステムが開発されている.このシステムはチェアサイドで簡便に自律神経機能を評価できるため,歯科診療中の生体反応のモニターとして有効と思われる.本研究は,重症心身障害者に対する歯科診療中に心拍変動解析を行い,障害者に対する歯科診療が自律神経機能に及ぼす影響を評価することを目的とした.  北海道大学病院小児・障害者歯科に通院中の重症心身障害者17名(男性6名,女性11名)に対して,歯科診療中に心拍変動解析を用いて自律神経機能の評価および心拍数(HR)の測定を行った.診療内容は①超音波スケーラーによるスケーリング,②歯ブラシによる口腔内清掃とし,各診療内容につき10名ずつ解析を行い,診療開始前,診療中および診療終了後の3つのphaseについて各評価項目の平均値を算出してその変化について検討を行った.統計学的分析は,Friedman testを用い,p<0.05で有意差ありとした.本研究は北海道大学病院自主臨床研究審査委員会の承認を得て行った.  その結果,心拍変動解析を用いた自律神経機能の評価は重症心身障害者の歯科診療中においても可能であった. スケーリング中のLF/HFは診療開始前,診療終了後と比較して有意に高い値を示した.口腔内清掃中のLF/HFは3つのphase間で有意な差は認められなかった.LF/HFはストレスとの関連が報告されているため,スケーリングがストレッサーとなっている可能性が示唆された.  本研究により,重症心身障害者の歯科診療中において,心拍変動解析を用いた自律神経機能の評価が可能であり,超音波スケーラーによるスケーリングによりLF/HFの上昇,ならびに歯ブラシによる口腔内清掃によりLF/HFが上昇する傾向がみられることが示された.
To provide safe dental practices, the monitoring biological reactions is important. The evaluation of autonomic nervous function using heart rate variability analysis is effective to assess a stress status in a non-invasive manner. The purpose of this study was to evaluate changes in autonomic nervous function among patients with severe motor and intellectual disabilities (SMID) during dental practices (ultrasonic scaling and tooth brushing). Subjects comprised of 17 patients with SMID. Heart rate variability analysis and heart rate were measured in 10 patients with SMID during ultrasonic scaling and tooth brushing, using the monitoring system for evaluating autonomic nervous function in real time as follows : R-R intervals of heart rate were divided into high-frequency (HF) and low-frequency (LF) components by power spectrum analysis. HF reflects parasympathetic nervous activity and LF/HF reflects sympathetic nervous activity. The results showed an autonomic nervous function was able to be evaluated in patients with SMID during dental practices in real time. In patients with SMID, the values of LF/HF and HR were higher during dental practices (ultrasonic scaling and tooth brushing). The values of LF/HF during ultrasonic scaling were significantly higher than both before and after treatment. LF/HF is related to stress, and these results suggest that dental practices were a stressor among patients with SMID. It appears possible to easily and non-invasively evaluate changes in autonomic nervous function among patients with SMID during dental practices in real time. The values of LF/HF tended to increase during dental practices.
