側頭下窩に生じた炎症性偽腫瘍の１例

Abstract
炎症性偽腫瘍は，炎症性細胞浸潤と線維化を主体とする腫瘍類似性病変である．今回，われわれは側頭下窩に生じた炎症性偽腫瘍の１例を経験したので報告する．症例は73歳女性．開口障害を主訴に，当科を紹介受診した．初診時の自力無痛開口量は16mmで，右頬骨下稜に硬結を伴う腫瘤性病変を認めた．CT，MR画像からは右頬骨下稜から側頭下窩に広がる軟組織病変を認めた．全身麻酔下に生検を行ったところ，病理組織学的には, 炎症細胞の浸潤のみられる線維性瘢痕組織であった．以上より右側頭下窩偽腫瘍と診断し，ステロイド療法を開始した．投与開始から６週間経過した時点で，腫瘤は消失し開口量は48mmとなった．
In this study we report on a case of inflammatory pseudotumor of the infratemporal fossa. A 73-year-old woman was admitted to our hospital because of trismus. CT and MR findings revealed the right infratemporal fossa mass had invaded the adjacent soft tissue. We suspected a malignant tumor of the right infratemporal fossa and performed an open biopsy under general anesthesia. Histopathological findings were we discovered fibrous scar tissue with inflammatory cell infiltrate. The patient was diagnosed with inflammatory pseudotumor in the right infratemporal fossa, and oral prednisolone was given for 6 months. The patient’s symptoms, including trisumus, improved dramatically with reduction in lesion size.
