口腔扁平苔癬の悪性化に関する免疫組織学的検討

Abstract

口腔扁平苔癬(以下OLP)は炎症を伴う難治性の口腔白色病変で,稀に扁平上皮癌への悪性転化がおこり問題となる.OLPの誘因は機械的刺激や歯科材料などが知られており,上皮下のリンパ球浸潤が上皮基底細胞に影響を及ぼしOLPが生じると考えらえているが,正確な病因は未だ明らかではない.また,OLPの癌化の機序にも諸説あり,未だに統一した見解は得られていない.本研究では,OLPの癌化の解明及び癌化予測因子を同定することを目的として研究を行った.1995年から2008年までの14年間に北海道大学病院を受診し,病理組織学的にOLPと診断された125例のうち54例を対象とし,臨床的背景因子(年齢,性別,発生部位等)及び,免疫組織学的検討を行った.上皮下炎症細胞について抗CD3,抗CD8,抗CD20抗体,上皮内樹状細胞は抗CD68抗体を用いて免疫組織学的に検索を行い,抗p53,Ki-67及び抗podoplanin抗体を用いて上皮の性状を検討した.54例のうち悪性転化した症例は3例であった.癌化した3例を癌化群,癌化しなかった51例を対照群とし検討を行った.臨床的背景因子では年齢,男女比に差はなかったが,発生部位において対照群では頬粘膜に好発することに対し,癌化群では舌に多くみられ,舌に生じたOLPでは,他部位に比べ癌化する傾向が高いことが示された.次に,癌化群3例と対照群51例を用いて病理組織学的に比較した.上皮下リンパ球は,CD20陽性細胞は少なくCD3陽性細胞が主体で,CD8陽性細胞が比較的多かった.CD68陽性細胞は上皮内に散在性に認められた.これら炎症に関連した細胞数や割合は,両群間に有意差が認められなかった.p53陽性細胞及びKi-67陽性細胞は癌化群で軽度に増加しているものの,その陽性率に有意差は認められなかった.podoplanin陽性細胞は癌化群で有意に多く(p<0.05),その分布にも差が認められた.podoplaninは癌化の初期には癌幹細胞が出現していると考えられることから,将来的に癌化するOLPではKi-67及びp53陽性細胞などの悪性化マーカーの発現に差の認められない段階でも, podoplaninが癌化の予測因子となりえることが示唆された.
Oral lichen planus (OLP) is a chronic inflammatory mucocutaneous disease and considered to be a potentially premalignant disorder. The pathogenesis of OLP is unclear, but subepithelial inflammatory reaction induced by stimuli including mechanical stress or dental materials have been thought to cause apoptosis of epithelial cells. The mechanism of malignant transformation of OLP is also controversial. The objective of this study was to evaluate the risk factors and to elucidate the mechanism of malignant transfomation of OLP. The target was 54 patients who had been pathologically diagnosed with OLP at Hokkaido University Dental Hospital. We investigated clinical background factors including age, gender and region. Immunohistochemical experiments were performed with anti-CD3, anti-CD8, and anti- CD20 antibody for subepithelial lymphocytes, with anti-CD68 antibody for intraepithelial dendritic cells and anti-p53, Ki-67 and anti-podoplanin antibody for epithelial cells. 3 cases were malignantly transformed in 54 cases. In the clinical background factors, although there was no difference among age or gender, OLP on the tongue seemed to have malignant potential. Immunohistological studies revealed that there was no difference of CD20, CD8, CD3, CD68, p53, and Ki-67positive cells between control and transformed groups. On the other hand, podoplanin positive cells and their distribution were significantly correlated with a risk of malignant transformation (p<0.05). These results indicated that aberrantpodoplanin expression and distribution correlate with malignant transformation of OLP and analysis of podoplanin staining on OLP could be a useful tool to predict termination.
