Abstract: 本研究では、家族や特別支援学校教諭などの看護師免許を持たない者に委託可能な技術範囲について検討することを目的に、重症心身障がい児(者)施設に勤務する看護師が行う気管吸引技術の範囲を調査した。研究方法は、看護師4名が実施する気管吸引技術とその関連技術の録画ビデオを言語化し、意味内容ごとに311の記録単位に分割した。結果、の5つのカテゴリーに分類され、看護管理と教育に関する看護行為は、委託すべきでない看護師の専門的業務と思われた。また、吸引技術の中には、小さな看護過程が存在しており、吸引技術の委託は同時にアセスメントの委託でもあった。現法では、家族が求められる技術範囲に制限はなく、特別支援学校教諭が許容された技術範囲よりも広範であった。特別支援学校教員に対しては、緊急時に最低限行うべき技術の設定が必要と思われた。今後の課題として、臨床工学技師をはじめとした医療職において合法的に吸引を行うための議論の必要性があると思われた。
This research aims to elicit key components of tracheal suctioning skills to establish effective delegating methods for unlicensed care providers (UCPs) who care for children with severe disabilities at home. The purpose of the research is to discuss proper range of delegatable nursing tasks for UCPs. 311 basic motions were abstracted from video-taped motion of four experienced nurses. Five categories were found. Of those, two categories relating to nursing management and education were considered as professional tasks of nurses which were undelegatable. Small nursing processes were also found in the process of care provision, which implies the delegation of tracheal suctioning also delegating small assessment at the same time. In current law, the range of skills that families require was wider than the special school teachers’. Serious discussion for health professionals, such as medical engineers to provide suctioning legally is a future theme.
