Abstract: 2014年2月,研究用のみならず臨床用の診断プロトコルとしてDC/TMD(Diagnostic Criteria for Temporomandibular Disorders)が発表された。これはRDC/TMDプロトコルの信頼性が向上するよう,エキスパートパネルら(members of the International RDC/TMD Consortium Network of the International Association for Dental Research(IADR), members of the Orofacial Pain Special Interest Group(SIG)of the International Association for the Study of Pain(IASP), and members from other professional societies)により大きく変更されたものであり,実際の妥当性(診断法の敏感度・特異度)の検証もなされていることが特徴である。結果DC/TMDプロトコルは最も一般的な疼痛関連顎関節症と顎関節内障における妥当な検査法と診断基準を含んでおり,臨床・研究どちらにもただちに応用するにふさわしい疼痛の生物心理社会的モデル(人間の心や体の病気は身体的要因,心の問題,生活環境や周囲との関係性のすべてがかかわっていると考えるモデル)に基づいた顎関節症患者の評価(検査)の集大成として今後の研究・臨床で利用されると思われる。本稿は,このDC/TMD診断プロトコル(原則,検査手法,診断方法など)をマスターするまでの過程について,小生が実際に学んだ手順と,DC/TMDの今後の展望について,RDC/TMDエキスパートパネルらのご意見やDC/TMD発表までの経緯,内情を交えながら紹介する。
In February 2014, DC/TMD (Diagnostic Criteria for Temporomandibular Disorders) was published as a clinical diagnostic tool as well as for research purposes. DC/TMD protocol underwent progressive improvement from RDC/TMD (1992) by a working group [consisting of members of the International RDC/TMD Consortium Network of the International Association for Dental Research (IADR), members of the Orofacial Pain Special Interest Group (SIG) of the International Association for the Study of Pain (IASP), and members from other professional societies] especially focusing on establishing its validity. The significant merit of DC/TMD was to obtain the sensitivity and specificity of each diagnosis of the most common TMDs (temporomandibular disorders). DC/TMD protocol contains both the proper clinical examination protocol and the diagnostic criteria of common pain-related TMD and joint problems. This classification system was based on the biopsychosocial model and the intent was to simultaneously provide a physical diagnosis and identify other relevant characteristics of the patient that could influence the expression and thus management of their TMD. The aim of this article is to introduce the steps of mastering DC/TMD. This article is based on my own experience studied through the subject's authorities (Professor Peter Svensson, Aarhus University, Denmark and Professor Thomas List, Malmo University, Sweden). Additionally, I would like to mention the future aspects of DC/TMD quoting the comments from the expert panels.
