| Abstract: | 近年，胆汁酸吸着レジンであるコレスチミドには，血中コレステロール値を低下させるだけでなく，血糖値を低下させる作用を併せ持つことが報告されている. 胆汁酸とエネルギー代謝との関連が明らかになってきており，コレスチミドの血糖降下作用のメカニズムにも胆汁酸分画の変化が関与している可能性もある. われわれは，外来通院中で高LDLコレステロール血症を伴う2型糖尿病患者19例に対しコレスチミドを12週間投与し，血中胆汁酸分画の変化と血糖降下作用との関連を検討した. 投与後，血中LDLコレステロール値の低下とともに，臍周囲径，空腹時血糖値，HbA_[1C]，HOMA-Rの有意な低下を認めた. しかし， 血中総胆汁酸濃度とその分画の濃度には変化を認めず，HbA_[1c] や臍周囲径の変化との相関関係も認められなかった.コレスチミドによる血糖降下作用は，血中胆汁酸分画の変化とは関連せず，他の機序が関与していることが示唆された. |
