Abstract:  本研究では相談機関に初めて来談した就学前幼児57名に対して,WPPSI-III,WISC-IVワーキングメモリー下位検査,自閉症スペクトラム障害(ASD)及び注意欠如多動性障害(ADHD)に関する保護者評定検査を実施した。保護者評定検査により群分けしたASDリスク群(N=16),ADHDリスク群(N=11),重複群(N=10),other conditions群(N=20)の認知プロフィールを報告した。全検査IQはASDリスク群及びADHDリスク群では平均域であったが,重複群及びother conditions群では低かった。また,それぞれの群で先行研究と類似する特徴的な指標プロフィールを示した。ADHDリスク群ではWISC-IVワーキングメモリー下位検査をバッテリーさせることでその特徴を描き出すことができ,また,WPPSI-III処理速度とASD特性が関連することを明らかにした。本研究ではWPPSI-IIIを臨床活用する際の臨床群ごとの認知プロフィールを提供するとともに,ワーキングメモリー検査と発達障害の困難に関する保護者評定をバッテリーさせることの臨床的有用性を確認した。
 The cognitive profiles of the Japanese version of Wechsler Primary and Preschool Scale of Intelligence-Third edition (WPPSI-III) in clinical samples has not been reported; furthermore, profiles of working memory assessed through Wechsler Intelligence Scale for Children-Fourth edition (WISC-IV) is also needed for its clinical application. In this study, we administrated the WPPSI-III’s 8 primary index subtests, the WISC-IV’s 2 working memory subtests, and parent-rating tests for autism spectrum disorder (ASD) and attention-deficit hyperactivity disorder (ADHD) on 57 preschool children who visited clinical institutions for the first time. Four clinical groups were grouped according to the parent-rating test : ASD risk (N = 16), ADHD risk (N = 11), the comorbidity of ASD risk and ADHD risk (N = 10), and the other conditions group (N = 20). As a result, the full-scale IQ scores of the comorbid group and the other conditions group were lower than that of the ASD group and the ADHD group. In ASD risk group, scores of verbal comprehension index and processing speed index were lower than the score of perceptual reasoning index. Moreover, in ADHD risk group, there were no differences between index scores, but working memory subtests had a lower score. Cognitive profiles of these clinical groups replicated previous research. Additionally, it revealed that the processing speed score is related to ASD rating scale score. Following these results, the implications for practice and the clinical utility of WPPSI-III were discussed.
